`/v1` is the versioned base; `/api` is a permanent alias for the same surface.

## Responses

Success is `{ "success": true, "result": …, "meta": { … } }`. Failure is
`{ "success": false, "error": { "code", "message", "detail"? } }`, where `code` is a closed set
(NOT_FOUND, METHOD_NOT_ALLOWED, VALIDATION_ERROR, EXECUTION_ERROR, RATE_LIMITED,
PAYLOAD_TOO_LARGE, INVALID_JSON, INTERNAL_ERROR) and `detail` is the tool's own code when it has one.

Every response carries `RateLimit-Limit`, `RateLimit-Remaining`, `RateLimit-Reset` and `RateLimit-Policy`; a 429 adds `Retry-After`.
